Forty-five days after Jose Mourinho criticized her for entering the pitch to help Eden Hazard during the final minutes of the game between Chelsea and Swansea City, former first team doctor Eva Carneiro seems to be on her way out of the club.

Sources close to the doctor and the team have revealed that the 41-year old Carneiro was summoned to take over her duties at the club's training grounds at Cobham last Friday, but she was a no-show, making it clear that she will not accept the terms of her new contract with Chelsea, ending her relationship with the Blues.

Carneiro was stripped of key duties after Mourinho accused her of making a mistake when she and team physiotherapist Jon Fearn rushed on to the pitch to treat Eden Hazard, having twice been waved on by referee Michael Oliver. "Mou" called her "naive", adding to his rant Carneiro showed "no knowledge of the game."

The Gibraltar-born doctor was part of the first team's staff since 2011.

Club officers have been led to know that the former first team doctor is seeking legal advice, since last week reports stated José Mourinho was under investigation for shouting "filha da p**a" at the doctor. If those claims end up being true, the Portuguese boss could be facing a five match ban.

According to people close to Carneiro, the doctor would be thinking about filing a lawsuit against the team, since she was not satisfied with the team's reaction to Mourinho's punishment.
